  • For new families, please note that completion of this form does NOT guarantee placement in our program.
  • Please make sure to list 2 different references: 1 FAMILY reference for your family from church staff or small group leader and a DIFFERENT STUDENT reference for your children (teacher, coach, small group keader at church etc).
  • As part of your application, please submit the 4 Registration Documents.
