3. Print and Fill Out the Registration Form .


4. Write a check for $30, payable to Jay Schwisow.


5. Send Policy Statement, Registration Form, and Check to:

6. Click the Sign Up link to become a web family member ($5/year web-fee is included within the $30/year registration fee.)
