Complete and sign all forms contained in the Adobe* pdf under the appropriate link below.  

1. Basketball Forms 

• Basketball registration was on Tuesday, October 20th, 2009.  Thanks to all that have registered. 
• First practice for boys is Monday October 26th at Julian Thomas.  B12 and B14 from 5:30-6:45pm with High School boys to follow.  There will be a parents meeting between the practices to introduce coaches and to distribute equipment.  Board members will be there to answer any questions. 
• You may bring your registration forms and fees to the first practice.  Players WILL NOT BE ALLOWED to practice without these forms being submitted.    
• For those who missed the registration please download the Basketball Forms, fill them out completely and mail to Paul Hugasian along with payment to the address listed below. 

2.   Volleyball Forms 

Issue a check for the total amount due made payable to LCHSA. Players will not be allowed to practice or play games until payment has been received by the board.
