CCHM RENEWAL PROCESS

FOR RENEWAL PURPOSES ONLY

This is the 2nd year that the CCHM renewal process will be paperless and completed mostly on the website.  In an effort to insure everyone is has an opportunity to read the CCHM Guidelines and Policies, including our Statement of Faith, these documents are included as part of the renewal requirement.  If you completed these for the 2008-2009 year, you do not need to review them again, HOWEVER, they are still in effect.

Please note that there is a new form to complete for renewal purposes.  Directions are listed below.

As you read through this information, you will notice some other changes in our renewal/registration process.  Please understand that the CCHM Leadership Team prayerfully considered these changes and implemented them to better serve our families and improve our website by utilizing all of its functions. 

As a renewing member, you need to complete the following tasks: 

1.  Update your profile (upon trying to login, you will be taken immediately to your profile).  Be sure to update phone numbers and email addresses.  If you are joining CCHM with FPEA membership, it is helpful if you provide any existing FPEA number you have, but it is not required; and

[We strongly recommend checking the options that allow you to receive a any of the available daily digest by email.  This will allow you to be updated with the latest information going onto the website.  This may include, but is not limited to informal get-togethers organized by our members and new items in the classified section.]

2.  Pay the registration fee no later than August 31 or by any scheduled field trip in August for which you want to register.  You may use the PayPal option contained within this online registration or mail/drop your payment to Calvary Chapel of Melbourne (see directions below); and

Benefits Only Membership - $27.00 for the year

Benefits & FPEA Membership - $45.00 for the year

3.  Submit a family photo no later than August 31 if you did not provide one for the 2008-2009 year.  You may send this by email to cchm@calvarymelbourne.org - remember to reduce the size of your digital picture - or provide a regular photograph with your registration fee if you mail/drop it to Calvary Chapel of Melbourne; and  

4.  Read and complete these 4 forms found on the navigation menu to your left once you login to the website private pages.  The bottom 3 forms were required for the 2008-2009 school year.  If you completed them for last year, you may disregard this notice.  The top form "Membership Survey" is new and must be completed for the 2009-2010 year

Payment Directions:  All checks must be made payable to Calvary Chapel of Melbourne and NOT Calvary Chapel Homeschool Ministry or CCHM.   You may mail your payment and family photograph(s) to Calvary Chapel of Melbourne, ATTN:  CCHM, 2955 Minton Road, Melbourne, Florida 32904.  You may also place these items into a ministry envelope, located at the Calvary Chapel of Melbourne church office, complete the requested information, and then drop it into the wooden tithe box.
