GROOVY Fall Teen Dance

All YHSA Teen Members and their Guest ages 13-19 are invited to attend the Groovy Fall Dance. (Each YHSA Teen can bring 1 guest)

Cost:  $10.00 per person

Includes: Pizza, Snacks, Snow-Cones, Fruit, Water Bottle w/ drink stix, Music, and a special Photo Spot (bring your own camera or cell phone and take photos)

REGISTRATION is open August 10th through October 5th.

There are 200 spaces available for this dance. Don't delay in registering, or you may not be able to reserve your spot.

Guest (Nonmembers) will register themselves online!!!

No Teen Member or their Parent is to register their Guest.

If a teen member or their parent registers their Guest, the registration will be DELETED and he/she will have to reregister them self again.

To register: FIRST, read the Dress Code, Code of Conduct, and Parent Medical Release at the bottom of this info page. Teens will have to agree to abide by both codes and parents will have to agree to the parent medical release. You will be asked to indicate your agreement by clicking YES in the electronic signature boxes on the sign-up form, and you WILL NOT have the chance to return to this page to reread the codes once you've begun your registration. (It might help to print this information before registering)

Once you have read both of the codes and the parent medical release, click on the "sign-up" link below to fill out the online form. Please note that the fields titled "First Name" and "Last Name" are for the Parent's Name. (These fields are generated by the website and cannot be changed.). Then continue filling in the rest of the form with the Teen's information.

TEEN MEMBERS: (Make sure you are logged in before you begin your registration process!!) On the 2nd page of the online registration form, you will need to put YOUR NAME when asked "Teen Member's Name".

When the form is submitted, you will be automatically directed to the payment page where you can make your payment.

TEEN MEMBERS, if you prefer to mail a check, make your check out to YHSA and write your membership # and "Fall Dance" on the memo line and mail your check to: Kristin Crizer, 308 Throne Road, Fawn Grove, PA 17321.

GUESTS, you will need the YHSA Teen Member's Name and Membership # when you register. Also, if you choose to pay by check, make the check out to YHSA and write "Guest of" followed by the Teen Member's Name and Membership # on the memo line. This will ensure that your registration is processed properly. Mail your check to: Kristin Crizer, 308 Throne Road, Fawn Grove, PA 17321.

IF a Teen Member is going to pay for their guest, the Guest will register them self and NOT make a payment.  The Teen Member will register them self and when he/she gets to "Price Option", he/she will click - "Fall Teen Dance-YHSA Member & Guest" for payment. The Teen Member will then enter their Guest's name when prompted "If PAYING for a Guest, enter Guest's Name:".

ALL PAYMENTS MUST BE MADE BY FRIDAY, OCTOBER 12TH

***Fall Dance Dress Code***

We ask that all attendees honor God by adhering to the highest standards of modesty and respectability in their dress for the dance. If any part of your outfit is questionable, please refrain from wearing it to the dance. Please note the following restrictions: Clothing that exposes the midriff area is not permitted. If you wear any type of spandex pants, your hips must be covered by a blouse or top that comes low enough to cover your rear. Please refrain from wearing any ripped jeans that expose undergarments. Shirts must cover the chest area properly. NO tank tops. Shorts should be no shorter than mid-thigh and skirts or dresses must be at or below the knee. If your skirt or dress is above the knee, then you need to wear leggings. Clothing should not display any offensive language, picture, or message.

***Any Teen found to be in violation of this dress code at check-in, will not be granted admission to the Fall Teen Dance until the violation has been corrected. Thank you in advance for your help with this!!***

***Code of Conduct***

All teens attending this event, including guests, will conduct themselves in a Christ-like manner. This means obeying all adults and chaperones, respecting the property of Action Church, and treating other attendees with respect and dignity. The use of tobacco, alcohol, illegal drugs, and profanity are expressly forbidden on the grounds of Action Church. No minor will be permitted to leave the event before 10:00pm unless signed out by a parent or guardian.

***Violating the Code of Conduct will be grounds of immediate expulsion from the event.***

***Parent Medical Release***

I hereby give YHSA and its representatives permission to seek emergency medical treatment for my child (as named on the online registration form) should it become necessary while he/she is attending the YHSA Fall Teen Dance. I have included a phone # where i can be reached in the event of an emergency. I release YHSA and its representatives of any liability while my child is at the YHSA Fall Teen Dance.
