Each individual student/guest attending prom must fill out their own registration form and make payment seperately to complete prom registration.

Dress Code is Formal
Any questions or clarification - ask WEST
Girls
- ALL dress must cover ALL cleavage - no exceptions!
- Practice leaning over to make sure your chest remains covered and not exposed
- Practice dancing in your dress to make sure everything stays in place
- Pins, dress tape, sweaters, shawls and tightening of straps are your friends!
- Back of dress must drop no lower than the midback area
- Tops and bottoms must overlap so midriff skin is not exposed
- Length of dress must be no higher than 3 inches above the knee
Guys
- Dress pants/Nice pants/Suits/Tuxedos - no blue jeans, no shorts, no rips, no tears
- Dress shirt/Collared button down shirt - no rips, no tears
- Tie
- Belt
Provision for infractions:
- Students will be offered an XL t-shirt by chaperone that they will need to wear
- This will make it possible for students to still attend the prom
- Consequence for not wearing t-shirt when requested by chaperone: Student will be escorted from prom

Code of Conduct
This code of conduct is based on 5 premises:
- Treatment of other people (fellow students, tutors and staff) must always be respectful
- This facility and all property will not be abused
- Each student is responsible for his/her own personal possessions
- Each student is accountable for his/her own actions
- Each student’s dress and actions should encourage others to Godliness
Arrival
Student will arrive between 5:30 and 7:00 pm
Students MUST check in at entrance when arriving
Departure
Students will depart at Midnight
Students MUST check out with WEST Director before leaving event
Students will not be allowed to exit the building once entering until prom dance is over (11:15 pm) or bonfire is over (midnight)
Dinner
There is no assigned dinner seating
Dancing & Bodily Contact
Music and dance at the discretion of and approved by WEST
Prohibited Substances
No warning will be given in this category
Tobacco, drugs, alcohol, weapons, and fireworks are not allowed in the building or parking areas
Anyone discovered with possession of any of these items during the prom event will be immediately removed WITHOUT REFUND
Provision for Infractions:
- Students will be removed from event
- Proper authorities will called if necessary
- Parents will be notified

Photography
Students and parents are welcome to take photographs of the event
Candid photography is welcome
WEST photography is property of WEST for the use of memories and promotion

General Expectations
As a Christian organization, WEST is committed to respect the position of the church and/or any location and their facility which we are renting and not to engage in any activities that would bring scandal to the Church, location, and/or WEST. (2 required) |

WEST Liability
- All bags will be check in at the beginning of the event
- All bags will be returned at the end of the event
- Bags will not be gone through during the event
- WEST is not responsible for lost or stolen items
- WEST will do its best to ensure the safety and security of anything or anyone however, WEST is not held liable for injury, theft and the like
LEGAL WAIVER
I agree prior to participating, I and the minor participant (student), will inspect the facilities and equipment to be used. If I believe anything is unsafe, I will immediately advise the administrators of such conditions.
I acknowledge and fully understand that each participant will be engaged in activities that involve risk of injury which might result not only from their own actions, inactions or negligence, but the actions, inactions or negligence of others, the rules of conduct, or conditions of the premises or any of the equipment used. Further, that there may be risks not known to us or foreseeable at the time.
I assume all foregoing risk and accept personal responsibility for the damages following such injury.
I, intending to be legally bound, do hereby release, waive, discharge and consent not to sue WEST’s administrators, board, employees, tutors or volunteers of the organization, other participants and Church of the Open Door, all which are herein after referred to as “released” from any and all liability to each the undersigned, his or hers and next of kin for any claims, demands, losses or damages, on account of injury including death or damage to property, caused or alleged to cause in whole or part by negligence to the release of otherwise in connection with association or entry and/or arising in participation in activities led by WEST.
I hereby release all members of WEST of any and all liability resulting from medical treatment. I understand if medical attention is necessary and I am not present, WEST has my permission to call an ambulance to transport any family member I have listed above to the nearest medical facility for emergency medical treatment. I am responsible for all expenses incurred.
THE ELECTRONICALLY SIGNED HAS READ THE ABOVE WAIVER AND RELEASE, AND UNDERSTANDS THAT HE/SHE HAS GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGNS IT VOLUNTARILY. (2 required) |
