The following forms must be printed, completed, and returned to a FACE board member along with the registration fees by the due date. All new and returning members must complete the "Backround Clearances and Affirmation Form" and turn in any required paperwork to the secretary. These are located under "Backround Clearances" on the side navigation. A copy of this form is also included in the registration packet.
