You are about to create a profile for the UHC member directory.

You are NOT registering for an enrollment appointment OR enrolling your children at UHC by submitting this form. The only way to register for an enrollment appointment is by completing online pre-enrollment using the instructions on our website: upstatecoop.org/enrollment/. Once you have completed online pre-enrollment and have an enrollment appointment, you must submit the form below in order to gain a profile on our member directory. This must be done prior to your enrollment appointment or you will be unable to register your children for classes. This directory profile will be activated by our staff at your on-site enrollment appointment.
