By clicking the above registration button and completing this registration for the activity below, I agree I am conducting this transaction electronically and in so doing I am agreeing to the following:

Calvary Old Bridge Home Education Group Membership, Events, and Activities.

July 1, 2023, through July 1, 2024.

I / We agree that in consideration of said Calvary Old Bridge permitting the participants identified by this electronic registration to participate in the aforementioned activity, I / We hereby agree to indemnify and save harmless said Calvary Old Bridge and their officials, volunteers, adult chaperones, employees and agents against any and all claims for loss or liability incurred to the aforementioned registrants as a result of said activity.

In the event of a minor illness or injury (such as a cold, headache, scrapes, sprains, abrasions, and/or small cuts) I do authorize the leading Pastor (and/or his/her representative), RN, or EMT to make common remedies such as Tylenol, cough medicine, etc. available to my child in dosages appropriate for his/her age and to clean, bandage, or wrap wounds as necessary.  If I cannot be reached, I grant permission to any physician or emergency medical personnel to render medical treatment necessary.
