I release, indemnify, and hold harmless Pine Lake Covenant Church and Labyrinth Home Education Co-op, Inc., a Washington nonprofit corporation, including its officers, directors, associates, employees, and volunteers, from any and all claims for damages or injuries of any kind (except in cases of intentional misconduct) arising from, during, or related to the participation of myself, my family, or any students named herein in any activity sponsored or sanctioned by Labyrinth Home Education Co-op, Inc. This indemnification and hold harmless clause extends to any claims by visitors whom I request be allowed to participate in or observe any activities sponsored by Labyrinth Home Education Co-op, Inc.
I agree to abide by any policies or guidelines that Labyrinth Home Education Co-op or Pine Lake Covenant Church may establish to mitigate the spread of COVID-19.
If this DISCLAIMER OF LIABILITY includes the release of any claims for injury suffered by a minor, which may subsequently be asserted by the minor (or the minor’s guardian, heirs, successors, or assigns), then I PERSONALLY agree to defend such claims and to cover all attorney fees and costs reasonably associated with the defense. I also agree to indemnify and hold harmless Pine Lake Covenant Church and Labyrinth Home Education Co-op, Inc., its officers, directors, associates, employees, and volunteers from and against any such claims brought by said minor for damages.
Additionally, I agree to pay for any damages to Pine Lake Covenant Church for which my family is responsible.
