CHILDCARE REGISTRATION 

Childcare (5th grade and younger)  Friday evening session only

 Cost is $6 per child paid upon registration. Refunds given minus processing fee if requested by January 15.  

No refunds can be given after January 15.

Nursing babies welcome to join parent. 

Please list the first and last name AND the age of each child (5th grade and younger) you need childcare for.

Indicate NONE if applicable.

 TEEN (13 and older) children also attending on Saturday (no teens Friday please)? Please fill out a separate registration form with payment for each. PLEASE NO CHILDREN UNDER 13 (unless it is a nursing baby).

TEENS AGES 13-17 MUST BE WITH PARENT.
