Alternate Monitoring

Contract for Opting-Out of Required Monitor Slots:

In lieu of the required background check (“opting out”), parents pay a fee of $10 per required monitor slot in addition to all other class and registration fees. These fees would be pooled and set aside to be used for paying others (“contracted monitors”) that have completed the proper requirements needed to comply with the state law for working in a volunteer position in contact with children. As long as there are sufficient contracted monitors available, paying parents would not be required to find someone to fulfill their monitoring slot; Cornerstone would assume the responsibility of filling those slots.

1 class = 4 slots, 2 classes = 8 slots,  3 classes = 10 slots, 4 or more classes = 12 slots (maximum required)

I hereby agree with the above statements and wish to use the alternate monitoring procedure and not complete the background checks at this time. I understand that if I change my mind I will need to complete all the components of the child protection law before monitoring. I also agree to pay for my necessary monitoring slots with my tuition (separate check please to make accounting easier).

Contract for Contracted Monitors:

Paid monitors will sign the portion below assenting to accepting and faithfully following through with all required accountability and oversight responsibilities according to the needs of Grace Baptist Church and Cornerstone, when fulfilling their time as a monitor. This would include confronting, in a godly manner, any situation deemed to be an infraction of the rules and regulations set up by both organizations and reporting, as needed, any said infraction that requires further attention. Failure to do so would be reason for dismissal from the fee monitor program.

Cornerstone would not be responsible for tax deductions and other federal and state fiscal requirements because the fee monitors would be considered “contractors” and therefore fees would be of a self employed nature, hence the need for a signed form of assent of duties.

I hereby agree with the above statements and would like to be added to the paid monitoring pool. I understand that I must have all the components of the Child Protection Law completed and kept up to date. I also understand that I must do my own monitoring responsibilities without compensation in addition to doing pool hours.
