"Enchanted Forest "

All YHSA Teens and their Guests ages 13-19 are invited to attend the Spring Formal!!! (Each YHSA Teen can bring up to 2 Guests.)

COST: $52.00 per person

Includes: Dinner Buffet (Tossed Salad w/ 2 dressings, Roll w/ butter, Baked Ham with Pineapple, Slow Roasted Turkey with Gravy, Baked Potato with Sour Cream and Chives, Glazed Baby Carrots, Penne Alfredo), Dessert Station, Tea, Lemonade & Water, DJ, Door Prizes,and more!!

 *The committee has made the menu as gluten free/dairy free as possible. No special dinners will be offered.*

Registration is open JANUARY 17, 2022 through March 14, 2022

Guests (Nonmembers) will register themselves online!!

NO Teen Member or their Parent is to register their Guest.

If a Teen Member or their Parent registers a Guest, the registration will be DELETED and he/she will have to register him/her self again.

To register: FIRST, read the Dress Code, Code of Conduct,, Parent Medical Release , and Covid Waiver at the bottom of this info page. Teens will have to agree to abide by both codes and parents will have to agree to the parent medical release and covid waiver. You will be asked to indicate your agreement by clicking YES in the electronic signature boxes on the sign-up form. Please take these consent forms very seriously. Clicking yes on the registration act as your signature and will be used as such if an issue should arise. For a printable form of each of the documents, simply click on the hyperlink for that document.

Once you have read both of the codes and the parent medical release, click on the "sign-up" link below to fill out the online form.

TEEN MEMBERS: MAKE SURE YOU ARE LOGGED IN BEFORE YOU BEGIN YOUR REGISTRATION PROCESS!!

TEEN MEMBERS: On the online form, you will need to put YOUR NAME when asked "Teen Member's Name".

When the form is submitted, you will automatically be directed to the payment page where you can make your payment.  

                If you prefer to mail a check,make your check out to YHSA and write your Membership # and "Spring Formal" on the memo line and mail your check to: Dawn Meeks, 3444 Shaffers Church Rd, Seven Valleys, PA 17360.

GUESTS, you will need the YHSA Teen Member's Name and Membership # when you register.

GUESTS, you will make your own payment and when the form is submitted, you will automatically be directed to the payment page where you can make your payment. Also, if you choose to pay by check, make the check out to YHSA and write "Guest of" followed by the Teen Member's Name and Membership # on the memo line. This will ensure that your registration is processed properly. Mail your check to: Dawn Meeks, 3444 Shaffers Church Rd, Seven Valleys, PA 17360.

IF a Teen Member is going to pay for their Guest, the Guest will register them self and NOT make a payment. The Teen Member will register them self and when he/she gets to "Price Option", he/she will click - "Spring Formal-YHSA Teen Member & Guest" for payment. The Teen Member will then enter their Guest's name when prompted "If PAYING for a Guest, Enter Guest's Name".

***ALL PAYMENTS MUST BE MADE BY MARCH 18, 2021 (Friday)***

Refunds will be provided very sparingly for those who are sick the day of the event or for unforeseen circumstances. Only those who are sick the day of the event will receive a full refund, including the fee PayPal charges us. Others will receive the refund minus the PayPal fee.

***Spring Formal Dress Code***

***Reminder from last year, SHOES must be worn at ALL times during the dance (NO bare feet). Girls, if you do not want to wear your high heels all night long, then you must bring another pair of comfortable shoes.***

Males attending shall dress to high standards of modesty and respectability in apparel ranging from semi-formal attire (includes dress pants, a button-down shirt & tie), to formal (i.e., a tuxedo or dress blues).

Females attending shall dress in formal attire that is cut for a woman and adheres to high standards of modesty and respectability. All gowns must be mid-calf length or longer, and any slits in the skirt must be at or below the knee. Skirts must be made of a solid material: gauze, tulle, or any see through material will not be permitted (they will only be acceptable as an overlay to the dress). All dress styles are permitted except for those that bare midriff, have cut out sides or backs, or show front or side cleavage. The back of the dress CANNOT exceed more than 5 inches below the shoulder blades (the part that bumps out on your back). Two-piece dresses MUST have a solid colored material under the two-piece dress (camisole top). If the dress has a corset back, it must have a solid colored panel behind it. Lace must be on solid, colored fabric (no flesh color). As an alternative, a woman's pant suit, a woman's dress pants with a fancy top (that is of code) are permitted to be worn (no men's tuxedos or other men's apparel should be worn).

If you are uncertain about attire,speak to a Spring Formal Committee Member for approval.

We want to represent YHSA and what we stand for in our bylaws. Please respect them. If you don't adhere to the rules, you will be asked to fix any code violations or call your parents. We will have safety pins & tank tops for you to use. We love our families and kids & want to guide them the best we can.

***Code of Conduct***

All teens attending this event, including guests, will conduct themselves in a Christ-like manner. This means obeying all adults and chaperones, respecting the property of Wisehaven Grand Ballroom, and treating other attendees with respect and dignity. The use of tobacco, alcohol, illegal drugs, and profanity are expressly forbidden on the grounds of Wisehaven Grand Ballroom.

**Violating the Code of Conduct will be grounds of immediate expulsion from the event.**

PLEASE NOTE: NO refunds will be given! If you are denied admission to or are expelled from the event due to a code violation, then you will not receive a refund.

***No Minor will be permitted to leave the event before 10:00pm unless signed out by a parent or guardian.***

 

***Parent Medical Release***

I hereby give YHSA and its representatives permission to seek emergency medical treatment for my child (as named on the registration form) should it become necessary while he/she is attending the Spring Formal. I have included a phone # where I can be reached in the event of an emergency. I release YHSA and its representatives of any liability while my child is at the Spring Formal.


***Covid Waiver***

RELEASE OF LIABILITY AND ASSUMPTION OF RISK

The individual named below (the “Participant”) desires to enter the premises (any location where related programming or activities are occurring, whether Wisehaven or any other location, “the Premise”) used by York Home School Association (YHSA), a nonprofit corporation incorporated in the Commonwealth of Pennsylvania, for the purpose of participating in group classes, activities, or social gatherings offered by YHSA (collectively "Activity"). As lawful consideration for being permitted by the YHSA to be on the Premise or engage in any Activity, the Participant hereby agrees to all the terms and conditions set forth in this agreement ("Agreement").

1. The novel coronavirus disease, COVID-19 (“the Disease”), has been declared a worldwide pandemic by the World Health Organization and is reported to be extremely contagious. The state of medical knowledge is evolving, but the Disease is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and show no symptoms and therefore spread the Disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or 100% effective vaccine for COVID-19 (at the time of this waiver). Evidence has shown that COVID-19 can cause serious and potentially life-threatening illness and even death. The Participant is aware of the highly contagious nature of bacterial and viral disease, including, without limitation, the Disease, and the risk that the Participant may be exposed to or contract the Disease or another disease or illness (each an “Illness”), by being present on the Premises or engaging in any Activity. The Participant understands and acknowledges that such exposure or infection from the Disease or other Illness may result in serious illness, personal injury, permanent disability, death, or property damage. The Participant acknowledges that this risk may result from or be compounded by the actions, omissions, or negligence of others, including the YHSA employees, contractors, representatives, consultants, or agents. The Participant understands that while the YHSA may have implemented preventative measures to reduce the spread of the Disease and other Illnesses, the YHSA cannot guarantee that the Participant will not become infected with the disease or any other Illness while on the Premises or participating in any Activity, and that being on the Premises or participating in any Activity may increase the Participant’s risk of contracting the Disease or another Illness. Notwithstanding the risks associated with the Disease and other illnesses, the Participant acknowledges that he or she is VOLUNTARILY ENTERING THE PREMISES OR ENGAGING IN ANY ACTIVITY WITH KNOWLEDGE OF THE DANGER INVOLVED, THE PARTICIPANT HEREBY AGREES TO ACCEPT AND ASSUME ALL RISKS OF PERSONAL INJURY, ILLNESS, DISABILITY, DEATH, OR PROPERTY DAMAGE RELATED TO THE DISEASE OR ANY OTHER ILLNESS, ARISING FROM BEING ON THE PREMISES OR ENGAGING IN ANY ACTIVITY WHETHER CAUSED BY THE NEGLIGENCE OF THE YHSA OR OTHERWISE.

2. The Participant hereby expressly waives and releases any and all claims, now known or hereafter known, against the YHSA and any related officers, directors, employees, agents, affiliates, contractors, representatives, successors and assigns (collectively “Releasees”) on account of injury, illness, disability, death, or property damage arising out of or attributable to the Participant’s presence on the Premises or engaging in any Activity and being exposed to or contracting the Disease or any other Illness, whether arising out of the negligence of the YHSA or any Releasees or otherwise. The Participant covenants not to make or bring any such claim against the YHSA or any Releasees, and forever releases and discharges the YHSA and all other Releasees from liability under such claims.

3. The Participant will comply with all instructions, orders, directives, and guidelines of the YHSA while on the Premises or while participating in any Activity, including, without limitation, requirements related to hand sanitation, social distancing, and use of face coverings. The Participant also agrees not to enter the Premises or participate in any Activity if the Participant is experiencing symptoms of the Disease or another Illness (such as cough, fever, fatigue, loss of taste or smell, sore throat, shortness of breath, vomiting, or diarrhea), has a confirmed or suspected case of the Disease or another Illness, or has come in contact in the last fourteen (14) days with any person who has been confirmed of having the Disease.

4. The Participant will defend, indemnify, and hold harmless, the YHSA and all other Releasees against any and all losses, damages, liabilities, deficiencies, claims, actions, judgements, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including attorney fees, and the cost of enforcing any right to indemnification under this Agreement, and the cost of pursuing any insurance providers, incurred by or awarded against the YHSA or any other Releasees, arising out of or resulting from any claim of a third party related to the Disease due to the Participant’s engaging in any Activity or being on the Premises.

5. The Participant acknowledges that this Agreement is binding and has been freely and voluntarily entered into without any inducement, assurance, or guarantee. Further, it is the Participant’s intent that this instrument act as a complete and unconditional release of all liability to the greatest extent allowable by law. 6. This Agreement constitutes the sole and entire agreement among the YHSA and the Participant with respect to the subject matter contained herein and supercedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter. If any term or provision of this Agreement is invalid, illegal, or unenforceable in any jurisdiction, such invalidity, illegality, or unenforceability shall not affect any other term or provision of this Agreement or invalidate or render unenforceable such term or provision in any other jurisdiction. This Agreement is binding on and shall inure to the benefit of the parties and their respective successors and assigns. All matters arising out of or relating to this Agreement shall be governed by and construed in accordance with the internal laws of the Commonwealth of Pennsylvania without giving effect to any choice or conflict of law provision or rule (whether of the Commonwealth of Pennsylvania or any other jurisdiction). 

Please be advised that, although this release mentions certain safeguards that YHSA could reinforce, our orgazation does not intend to enforce face covererings or anything of that manner at this time.
