2018 YHSA Teen Spring Formal

"Rolling Out the Red Carpet"

YHSA Teens are allowed to bring up to 2 guests. All Guests' ages must be from 13 to 19.

Cost: $48.00 per person

Includes: Dinner Buffet (Tossed Salad w/ 2 dressings, Roll w/ butter, Chicken Parmesan, Baked Honey Ham w/ Sweet Crushed Pineapple, Mashed Potatoes, Corn, Mac & Cheese), Dessert Station (Cream Puffs, Eclairs, Cookies, Brownies, Fudge), Tea & Lemonade, DJ, Door Prizes, Photo Booth, and more!

Registration opens January 16, 2018 through March 5, 2018.

There are 285 spaces available for this dance.

To register: FIRST, read the Dress Code, Code of Conduct, and Parent Medical Release at the bottom of this info page. Teens will have to agree to abide by both codes and parents will have to agree to the Parent Mecial Release. You will be asked to indicate your agreement by clicking YES in the electronic signature boxes in the sign-up form, and you WILL NOT have the chance to return to this page to reread the codes once you've begun your registration.

Once you have read both of the codes and the parent medical release, click on the "sign-up" link below to fill out the online form. Please note that the fields titled "First Name" and "Last Name" are for the Parent's Name. (These fields are generated by the website and cannot be changed.) Then continue filling in the rest of the form with the Teen's information.

***Your YHSA Teen Member will be making your payment for the YHSA Teen Spring Formal.***

You will need to pay your Teen Member directly

***ALL PAYMENTS MUST BE MADE BY SATURDAY, MARCH 17, 2018***

(Once you have completed your registration and you have received your Registration Confirmation, contact your Teen Member ASAP so that they can make a payment. Registration isn't complete until payment has been made.)

You will need your Teen Member's Name and Membership # before you begin your registration.

***Spring Formal Dress Code***

Males attending shall dress to high standards of modesty and respectability in apparel ranging from semi-formal attire (includes dress pants, a button down shirt & tie) to formal (i.e., a tuxedo or dress blues).

Females attending shall dress in formal attire that is cut for a woman and adheres to high standards of modest and respectability. All gowns must be mid-calf length or longer, and any slits in the skirt must be at or below the knee. Skirts must be made of a solid material: gauze, tulle or any see through material will not be permitted (they will only be acceptable as an overlay to the dress). All dress styles are permitted except for those that bare midriff, have cut out sides or backs, or show bront or side cleavage. The back of the dress CANNOT exceed more than 5 inches below the shoulder blades (the part that bumps out on your back). As an alternative, a woman's pant suit, a woman's dress pants with a fancy top (that is of code) are permitted to be worn (no men's tuxedos or other men's attire should be worn).

**Any teen found to be in violation of this dress code at check-in, will not be granted admission to the formal until the violation has been corrected.**

***Code of Conduct***

All teens attending this event, including guests, will conduct themselves in a Christ-like manner. This means obeying all adults and chaperones, respecting the property of Wisehaven Grand Ballroom, and treating other attendees with respect and dignity. The use of tobacco, alcohol, illegal drugs, and profanity are expressly forbidden on the grounds of Wisehaven Grand Ballroom. No minor wil be permitted to leave the event before 11:00pm unless signed out by a parent or gaurdian.

**Violating the Code of Conduct will be grounds of immediate expulsion from the event.**

PLEASE NOTE: NO refunds will be given! If you are denied admission to or are expelled from the event due to a code violation, then you will not receive a refund.

***Parent Medical Release Form***

I hereby give YHSA and its representatives permission to seek emergency medical treatment for my child (as named on the registration form) should it become necessary while he/she is attending the Spring Formal. I have included a phone # where I can be reached in the event of an emergency. I release YHSA and its representatives of any liability while my child is at the Spring Formal.
