Pre-PACE Testing Self-check Screening

For the safety of our members as we use the church facility as a PACE member you agree to follow these guidelines.

  • Temperature check will be done on anyone entering the building.
  • Hand sanitizer available at all PACE entrances for anyone to use as they enter the building.
  • Masks are required for everyone older then 2yrs old and kept on at all times while using our space indoors
  • Social distancing is to be observed at all times.

Please answer these self screening questions concerning SARS-CoV2/COVID-19 for each family member entering PACE facilities.

Have you had ..

#1: Fever or Chills in the last 48 hrs?

#2: Any of the following symptoms in the last 7 days?  Cough, New loss of taste or smell, Sore throat, Shortness of breath or difficulty breathing, Diarrhea, Nausea or vomiting, Stomachache, Tiredness, Headache, Muscle or body aches, 

#3: Past 14 days contact with anyone with symptoms?

#4: Past 14 days contact with anyone with Covid-19?

#5: Self isolating or quarantining?

#6: Waiting on the results of a COVID Test?

If you answered "Yes" to any of the self screening questions,please do not come to PACE at this time. Seek medical evaluation for SARS-CoV2/Covid-19.

Please do not come in until you meet our re-entry requirements for isolation of quarantine.
