FULL MEMBERSHIP: I/We do hereby affirm that I/we agree and comply with statements of faith and belief on family and on education.

ASSOCIATE MEMBERSHIP: I/We do hereby affirm that I/we agree and understand that the mission of CCHSA is to provide an organization of support that is in alignment with Christian values and principles as partially outlined in the statement of faith. Furthermore, I/we do hereby affirm that I/we have fully read the CCHSA statement of faith and fully agree to respect the member statement of faith in both word and action while participating in all CCHSA activities. I/We also agree to abide by the CCHSA code of conduct in all public meetings or gatherings related to CCHSA, including but not limited to: co-op classes, field trips, group activities and presentations. I/We understand that Associate Members do not have voting privileges and are not allowed to serve on the board.

LIABILITY RELEASE

In consideration for being accepted by CCHSA for participation in activities or trips, I do hereby release, forever discharge and agree to hold harmless CCHSA, its members, and the leaders thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever, which may be incurred by the undersigned and the participant, participants, or parents, that occurs while said person or persons are attending or participating in activities or trips arranged, hosted, or attended by members or leaders of CCHSA. The undersigned further hereby agrees to hold harmless and indemnify CCHSA, its members, and its leaders for any liability sustained by said acts of said participant, participants, or parents, including expenses incurred attendant thereof.
