HIGH SCHOOL GROUP LIABILITY RELEASE

I am allowing my child to participate in FEFC High School Group activities. As such, I fully understand and acknowledge that;

(a) risks and dangers exist in participating in some group activities including, but not limited to: boating, swimming, kayaking, dock activities, and a myriad of other activities either indoors or outdoors;

(b) my participation or my child's participation in such activities is entirely voluntary;

(c) participation may result in injury or illness including, but not limited to, bodily injury, strains, fractures, partial and total paralysis, eye injury, blindness, heatstroke, heart attack, death, or other ailments that could cause severe disability;

(d) by participating in these activities, I assume all risks, dangers, and responsibility for any losses and damages.

I, on behalf of myself, my representatives, and my heirs, voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify anyone associated with the high school group, its agents and officers, or parent volunteers from all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise out of my participation in the FEFC High School group. This waiver is valid throughout my student's participation in high school with FEFC events.

In consideration of my child being allowed to participate in FEFC events, I acknowledge and agree that participation includes possible exposure to and illness from infectious diseases, including, but not limited to, COVID-19, MRSA, and influenza. While particular rules and personal discipline may reduce this risk, the risk of serious illness does exist, and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown.

In the event of any such illness or disease, I agree to release, indemnify, and hold harmless FEFC fully and their officers, employees, staff members, volunteers, other families, and hosts from all liability, claims, and or litigation that myself or my child may have which are directly or indirectly related to my child's participation while at FEFC. I further understand that this document supplements, and in no way eliminates, all other documents, waivers, and releases signed on behalf of my child for their participation in FEFC events.
