Please read this step-by-step guide to requesting membership to Cornerstone Cooperative for 2024-2025.

 Please allow yourself 20-30 minutes to complete this process. DO NOT CLOSE OUT of the "Request Membership" process until you have completed all steps listed below. If you close out of any of the steps, your information will be lost, and you will need to request membership to Cornerstone Cooperative again.

Step 1: Complete the "Request Membership" page you are currently viewing. 

Step 2: Click the "Continue" button at the bottom of the page.

Step 3: On the next page, you will need to complete all 4 required forms listed below the message in red. DO NOT CLOSE OUT of this page until you have completed the "Family Information Form". If you close out of this page, your information will be lost, and you will need to request membership to Cornerstone Cooperative again. 

Step 4: First, you will need to click on "Family Information Form".  Please click "Submit" after completing the form.

Step 5: You will be routed to a page to choose the "Handbook Signature Page". Prior to completing this form, please review the 2024-25 Cornerstone Co-op Parent & Student Handbook that is linked on the "Handbook Signature Page". You may download the handbook to keep for reference.  Please click "Submit" after completing the form.

Step 6: Next, you will be automatically directed to complete the "Student Concerns Form".  As a new family, you will need to list all of your prospective students on this form, along with any concerns you have for each of them. Please click "Submit" after completing the form.

Step 7: The 4th and final form is the "Release of Liability".  Please click "Submit" after completing the form. 

Step 8: After submitting the "Release of Liability", your membership request will be sent to our Adminstration. If your forms are filled out completely, you will be contacted with details regarding the interview and registration process for 2024-25. 

Please fill out all required forms completely. If they are not complete, we will not be able to move forward with your interview process.
