Membership Registration
Fill out the form below and click the Continue button at the bottom.
Registration is a two step process.
The first step is to either
a.) Contact the JPGHE Membership Coordinator via email: jpghemembership@gmail.com and provide her with your phone number so she may call you back in person. Please indicate if she may text you to find a best time to call.
or
b.) complete the "REQUEST MEMBERSHIP" online form and submit it.
The second step is to fill out the additional membership forms as follows:
After you click Continue you will see a "Membership Registration" message . At the bottom of this message you will find a link to our current "JPGHE Membership Forms" which include permissions and waivers. Please click that link and fill out those additional forms.
You will not be able to log in to until those additional forms are submitted and the membership coordinator approves your request to join.
Our Membership Coordinator or her representative will contact you to aquaint you with our group and its activities, answer questions you may have, and get to know how we may serve you and your family.
Please note: Information provided on the online REQUEST MEMBERSHIP form will be used for the purpose of compiling
JPGHE Membership Rosters and a Membership Directory. The Membership Rosters and Membership Directory are to be used by JPGHE members only.
By providing information on this form, you agree that it may be added to JPGHE Membership Rosters and Member Directory.
Therefore, leave blank any spaces for information that you do not want shared with members of JPGHE.
Membership Directories, accessible only to current registered JPGHE Member Families, will be available online
for current Member Families’ own personal use. No other use (for solicitations, for example) of the online or
printed Membership Directory or Membership Rosters is allowed.
