Terms and Conditions

By registering my child for classes at PALS Enrichment:

1. I have read and agree to abide by all PALS Enrichment Policies.
2. I have read and agree to abide by all MAUMC Rules for Use.
3. I certify that I am the parent or legal guardian of the minor child(ren) listed below and voluntarily consent to their participation in PALS Enrichment Program. This agreement applies to all associated activities and events attended.
4. I recognize and agree to assume the full risk and responsibility for any and all injuries, damages, or losses regardless of severity which I, my minor child(ren), or my guests may sustain as a result of participating in PALS Enrichment Program and any and all associated activities.
5. I, on behalf of myself, my child(ren), and my guests, voluntarily release, waive, and hold harmless PALS Enrichment Program; its current and former directors, officers, board members, teachers, volunteers, members, and host facilities from any and all claims, damages, losses, liabilities, costs, or expenses arising from or related to participation in PALS Enrichment Program and associated activities.
6. I recognize and agree to waive and relinquish all claims I, my minor child/ward, or my guests may have as a result of participating in the program against the officials, organizers, agents, associates and employees.
7. I have read and fully understand the above information to which I have affixed my signature when I created my account.
