Bridgeway Homeschool Support Group is a ministry of Bridgeway Christian Church. We are a community of families who desire to see their children pursue godly, Christian characteristics. Our goal is to support and encourage families through the sharing of current information, resources, and activities; provide social and educational experiences; and to create service opportunities that help others in need.

5. WAIVER OF LIABILITY BRIDGEWAY CHRISTIAN CHURCH:

Until revoked in writing, I hereby fully release Bridgeway Christian Church, religious corporation of the State of California, its trustees, staff, members of the Board, and/or any adult leaders, whether volunteer or professional, from all liability for any accident(s), injury(s), illness(es), and/or death caused to myself that may come from my voluntary participation in athletic, recreational, social, transportation and/or any other activity sponsored by Bridgeway Christian Church and the Bridgeway Homeschool Group. By submitting my registration fee, I agree that I fully understand this Waiver of Liability and it shall bind my heirs, executors, administrators, assigns and/or any other persons having control over my affairs.

6. MEDICAL RELEASE BRIDGEWAY CHRISTIAN CHURCH:

IN CASE I AM UNABLE TO SIGN OR GIVE VERBAL CONSENT IN AN EMERGENCY, I HEREBY AUTHORIZE the administration of any medical treatment deemed necessary by Bridgeway Christian Church Staff, Leadership of the Bridgeway Homeschool Group, and/or any physician licensed under the provisions of the Medical Practice Act on the staff of a licensed hospital.

IN CASE MY CHILD REQUIRES EMERGENCY MEDICAL TREATMENT I UNDERSTAND THAT I WILL BE CONTACTED IMMEDIATELY. I represent that I am the parent or Legal guardian of the Minor(s) named on this form.

IN THE CASE I AM UNABLE TO SIGN OR GIVE VERBAL CONSENT ON BEHALF OF THE MINOR(S) NAMED ON THIS FORM, I HEREBY AUTHORIZE the administration of any medical treatment deemed necessary by Bridgeway Christian Church Staff, leadership of the Bridgeway Homeschool Group, and/or any physician licensed under the provisions of the Medical
Practice Act on the staff of a licensed hospital.

7. PHOTO RELEASE BRIDGEWAY CHRISTIAN CHURCH:

In connection with my family’s membership in the Bridgeway Homeschool Support Group, it is my understanding that Bridgeway Homeschool Support Group and/or other members may take digital pictures at various activities and events and I understand that Bridgeway Homeschool Support Group may use the images in any media for any purpose which may include, among others, advertising, promotion, social media, marketing and packaging for any product or service. I agree that the images may be combined with other images, text and graphics, and cropped altered or modified.
