MEMBERSHIP ELIGIBILITY
Members must be currently providing at least 51% of instruction to at least one child within the home, through one of four options:

  1. Overseeing their own private schools in their homes (filing a private school affidavit)
  2. Enrolled in a PSP (private school satellite program)
  3. Enrolled in a public or private ISP (independent study program through a district or charter school)
  4. Using a tutor 

OR their oldest child is not yet compulsory attendance age (6 as of September 1).

REQUIRED PAPERWORK
Once you click on "Continue" or "Update" below, you will need to complete the following forms. These forms, along with payment of the membership fee, are required to join LIFE Homeschool. Your registration will not be approved and you will not be able to sign-up for field trips until these are complete.

Acknowledgement of Statement of Faith
LIFE Homeschool requires familiies sign an acknowledgement of our Statement of Faith to participate in our activities. Please note that this form must be signed by both parents.

LIFE Policy Handbook Form
Families should read through the LIFE Homeschool Policy Handbook and then complete this form, acknowledging that you have received, read through, and will abide by these policies. This form must be signed by both parents.

Adult Waiver of Liability
Please complete this form once for each parent. Any additional adults you have included in your registration must also complete this form prior to attending LIFE events.

Youth Waiver of Liability
If all the information is the same, you may include all of your children on one form. If you need additional Youth Waiver forms, please contact us.
