GRACE (Gainesville Region Alliance of Catholic Homeschoolers) is a community of Catholic Homeschoolers that exists to promote and support homeschooling and community among Catholic families. GRACE serves homeschooling families through fostering the Domestic Church, supplemental virtue-based education opportunities, and building a community rich in Catholic culture. We seek to serve through prayer, service, and community.

Our homeschool group was built around celebrating the Domestic Church. We start the year with a birthday party for Mary and then continue to celebrate the Saints every month. We have Saint of the Month classes (one a month) and Clubs (once a month). We offer Co-Op classes for both Elementary and Secondary (Middle and High School). We also offer in-house and off-site field trip and events. We are 100% volunteer, run by the families who participate, and there are involvement requirements.

1. ACKNOWLEDGMENT OF RISKS
I, the undersigned parent or guardian, do hereby acknowledge that participation in the Gainesville Region Alliance for Catholic Enrichment (hereinafter referred to as GRACE) involves certain inherent risks, dangers, and hazards for myself and my child/ren. These may include, but are not limited to, accidents, illnesses, personal injury (including, but not limited to, permanent disability or death), and property damage. I understand that the GRACE is run on a cooperative basis and relies on parent volunteers, and that not all risks can be entirely foreseen or eliminated. I grant permission for for my children to participate in physical activities at Holy Trinity Catholic Church in Virginia and at such other locations as may be designated from time to time under the auspices of GRACE.

2. RELEASE OF LIABILITY AND HOLD HARMLESS
In consideration of being permitted to participate in the GRACE and its related activities, I, on behalf of myself, my children, my heirs, executors, and assigns, do hereby release, waive, and forever discharge the Ganesville Region Alliance for Catholic Enrichment, its administrators, instructors, volunteers, agents, Holy Trinity Catholic Church and the Catholic Diocese of Arlington from any and all claims, demands, or causes of action arising out of negligence or otherwise, for any personal injury, property damage, or loss of any kind that I or my child may suffer during our participation in GRACE activities.

3. MEDICAL TREATMENT AUTHORIZATION
In the event of an emergency, I hereby grant permission to GRACE or Holy Trinity Catholic Church staff and volunteers to administer first aid or obtain necessary emergency medical treatment for my child(ren). I agree to assume full responsibility for all costs, expenses, and medical bills incurred as a result of such treatment.

4. INSURANCE DISCLOSURE
Gainesville Region Alliance for Catholic Enrichment carries liability insurance. ] I understand that it is my responsibility to maintain my own family’s health and accident insurance.

5. GOVERNING LAW AND SEVERABILITY
This Agreement shall be governed by and construed in accordance with the laws of the State of Virginia. If any provision of this document is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

I HAVE READ THIS RELEASE OF LIABILITY AND WAIVER AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING MY RIGHT TO SUE GRACE, HOLY TRINITY CATHOLIC CHURCH, THE DIOCESE OF ARLINGTON OR ITS VOLUNTEERS. I SIGN THIS AGREEMENT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

By signing this document, I acknowledge that if anyone is hurt or property is damaged during my child's participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against the provider and True Life Connection LLC on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read this entire document. I have read & understood it, and I agree to be bound by its terms. By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.

Volunteer Commitment and Continuity of Instruction

By signing up to teach, lead, or assist with a class or club, you are making a vital investment in our community. Our families pay registration and supply fees to attend these programs, and because these fees are non-refundable, our students and parents deserve a consistent, high-quality experience throughout the entire school year.

Full-Year Commitment: I understand that my commitment to volunteer or teach is for the entire school year. (Elementary Co-Op consists of 2 half-year sessions.)
Mid-Year Withdrawal & Replacement Policy: In the rare and unavoidable event that I must step down from my role mid-year, I agree to give the leadership team as much advance notice as possible. Furthermore, I agree to assist actively and to the best of my ability in finding and transitioning a qualified replacement leader or teacher to ensure continuity for the students. Furthermore, I agree to supply complete lesson plans to fulfill the remainder of the school year.

I acknowledge that the success of our program relies heavily on the dependability of our volunteers, and I agree to fulfill these expectations to the best of my ability.

Informed Consent to Medical Treatment

I request that in my absence the above-named minor(s) be admitted to any hospital or medical facility for diagnosis and treatment when a condition or injury arises that is serious enough that a reasonable person would seek care right away. I request and authorize physicians, dentists, and staff, duly licensed as Doctors of Medicine or Doctors of Dentistry or other such licensed technicians or nurses, to perform any diagnostic procedures, treatment procedures, operative procedures and x-ray treatment of the above minor. I have not been given a guarantee as to the results of examination or treatment. I authorize the hospital or medical facility to dispose of any specimen or tissue taken from the above named minor. I assume full responsibility for all costs of such treatment. Further, should it be necessary for the participant to return home due to medical, disciplinary, or other reasons, I do hereby assume responsibility for the participant’s transportation home and any costs related thereto. By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.

Payments for annual membership dues can be paid directly through Homeschool-Life. After logging into your account, select "Pay Invoice" from the main menu and complete your payment transaction.
