1. Emergency Contact & Phone Number Membership Agreement & Waiver of Release 2. I understand, by submitting this Learning Tree Homeschool Co-op application form that our family is participating in co-op activities AT OUR OWN RISK, and therefore we release Learning Tree Homeschool Coop, the Leadership Team, and Clover Pass Community Church (CPCC) from any and all liability. I admit and acknowledge that participation in co-op activities may possibly expose myself and/or my child(ren) to illness from infectious diseases. 3. I also understand that being a member of The Learning Tree Homeschool Co-op involves personal responsibility, and I will do my best to read the Parent Handbook as well as adhere to the guidelines set forth within. The Co-op Leadership team reserves the right to revoke any membership which they deem necessary. I understand that the Co-op Leadership Team and CPCC have access to my membership information as well as my background check/youth worker information. 4. I also understand and give permission for The Learning Tree Homeschool Coop to use pictures/ images of my family members or myself in our newsletter, website, and for all promotional materials. I understand that these pictures/images help to tell the story of homeschooling and I release and hold harmless CPCC, its photographers, agents, licensees, and employees from any and all claims and demands associated with this release, including all claims for libel. I also waive any claim for damages and any right to compensation that I may have for the foregoing. 5. Signature By typing my name below, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature.

Payment Instructions

To pay the registration fee*, please bring check or cash to the orientation meeting. You may also mail a check (made out to Clover Pass Community Church) to CPCC, Att: Learning Tree Co-op, 105 North Point Higgins Rd, Ketchikan, AK, 99901.

*Fall session $40, winter session $25, spring session $25
