We are honored you would like to request membership to Rooted and Rising Community.

All fields designated with an asterisk must be completed before the form can be submitted. Please carefully read through the policies, Statement of Faith and Fee Schedule.
Please include ALL children in the household who will be on campus on Community Day. This includes unenrolled siblings under the age of three. To add an additional child, select the (+) Add Child option.
Please pay the Enrollment Fee at the bottom of the application. Your application is not complete until the Enrollment Fee is paid.
Please enter the grade your child will be entering next year (not their current year).

Payment Instructions

Please pay the Family Enrollment Fee of $150 below (only credit card payments at this time). If you want to pay via check or cash, please email us at Hello@RootedRisingCommunity.com. For credit card payments, there is a pass through credit card fee of 3%.

You are not enrolled, nor are your student(s) spot held until the Enrollment Fee is paid.
