5. Grasp Homeschool Co-op occasionally takes photographs or videos during classes, activities, field trips, and events for the purpose of sharing with Grasp Co-op families and promoting Co-op activities on our website and social media platforms.

I grant permission for images of myself and/or my child(ren), captured during co-op through video, photo and digital camera to be used solely for the purpose of Grasp Co-op. I understand that the images may be used in print publications, online publications, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. YES – Permission Granted

6. GRASP Homeschool Co-op Release of Liability and Assumption of Risk

This Release of Liability and Assumption of Risk Agreement (“Agreement”) is entered into by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of themselves and the minor child(ren) listed below in connection with participation in the Grasp Homeschool Co-op.

Participation
Parent/Guardian grants permission for their child(ren) to participate in classes, activities, meetings, field trips, and other events organized or hosted by the Grasp Co-op.
Assumption of Risk
Parent/Guardian understands that participation in Grasp Co-op activities may involve certain risks, including but not limited to physical activity, outdoor play, field trips, use of equipment, and interaction with other participants. Parent/Guardian voluntarily assumes all risks of injury, illness, damage, or loss that may arise from participation.
Release and Waiver
To the fullest extent permitted by law, Parent/Guardian releases, waives, and discharges the Grasp Co-op, its organizers, leaders, teachers, volunteers, host families, property owners, and participants from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur during or as a result of participation in Grasp Co-op activities.
Medical Authorization
In the event of an emergency, Parent/Guardian agrees to be responsible for any medical expenses incurred.
Responsibility for Supervision
Parent/Guardian acknowledges that the Grasp Co-op is a cooperative community and that parents may be responsible for teaching, assisting, or supervising activities. Parent/Guardian agrees to supervise their child(ren) when required and to ensure that their child(ren) follow safety guidelines and instructions.
Use of Church Facilities
Parent/Guardian acknowledges that the Grasp Co-op may meet at or use facilities owned or operated by a church or other host organization (“Host Facility”). Parent/Guardian agrees to respect and follow all rules, policies, and guidelines established by the Host Facility.
Parent/Guardian further agrees that the Host Facility, its staff, leadership, and congregation are not responsible for supervising participants and shall not be held liable for any injury, illness, damage, or loss occurring in connection with Grasp Co-op activities. Parent/Guardian releases and holds harmless the Host Facility and its representatives from any claims or liabilities arising from the use of its property by the Grasp Co-op.
Indemnification
Parent/Guardian agrees to indemnify and hold harmless the Grasp Co-op, its volunteers, and the Host Facility from any claims arising from the actions of the Parent/Guardian or their child(ren) during Grasp Co-op activities.
Acknowledgment
Parent/Guardian acknowledges that they have read this Agreement, understand its terms, and sign it voluntarily.

Payment Instructions
