Olivet Private School Transcript Request Form

indicates a required answer

Transcripts are free for current students and are prepared for mailing or pick-up within two weeks upon receipt of complete authorized requests. Please submit a separate request for each address to which you want a copy sent.

For Alumni requesting transcripts, we can provide it for you or send it to the school of your choice. The form must be signed by the student/alumni, not the parent. Please submit a separate request for each address to which you want a copy sent.

Processing Fee schedule for Alumni is listed below (please plan accordingly):

  • $10 per mailing address - sent within 2 weeks of receiving request form.
  • Rushed - $25 per mailing address - sent within 7 days of receiving request form.

Payment must be received prior to sending out the transcript. Payment can be via PayPal, check or cash. Checks should be made out to Olivet Private School and checks/cash can be mailed/dropped off to 501 SE Port St. Lucie Blvd., Port St. Lucie, FL 34984. Please be sure to mark what this money is for. TRANSCRIPTS CANNOT BE ISSUED UNLESS ALL FINANCIAL OBLIGATIONS ARE SATISFIED.
