I understand that I am responsible for my child(ren)'s behavior, safety, and supervision during the time that he/she is attending BACH sponsored activities, and that it is my responsibility to be on site during activities sponsored by BACH, the exception being on site during a youth activity for the Jr. High and Sr. High ages. I have read and I have/will instruct(ed) my child(ren) in the appropriate behavior. 4. I understand it is the responsibility of the parent and not members of BACH or the staff or personnel of the facility used by BACH to attend to the medical condition or needs of my child(ren). 5. I understand that the payment of BACH membership dues is non-refundable. Paid dues and a notarized liability form, signed by both parents, will allow me access to the BACH Group website which gives me privileges to the group forums, activity calendar, files, and activities. 6. I understand that being a member of BACH brings with it the responsibility of being on a committee. I agree to follow through with the responsibilities the committee chairperson asks. I will communicate with and be available to the chairperson of my committee. 7. I understand that I am responsible for reading and understand all the information in the BACH Handbook, agree with the Statement of Faith, and agree to adhere to all BACH principles, beliefs, and guidelines.
