Rental Agreement

This states many of our written agreements with Sovereign Grace Bible Church. Please read it with your children, and include your signature and the signatures of all students 8 years and older.

1. When outside of the classrooms, students are to remain in the playground area only. Students should be attended by adults at all times.
2. Students should NOT:
   • Play, hang, or slide on the railings.
   • Play or sit on the stairs or ramps.
   • Play, sit, or stand on the outside block wall.
   • Play in the large ditch by the Sunday School Building.
   • Play among the cars or parking lot.
   • Sit in cars without parental supervision and/or permission.
   • Run around the buildings.
   • Have bicycles, skateboards, or skates on campus.
   • Retrieve play equipment that goes off campus without permission from a parent teacher.
   • Sit on outside stairs without strict parental supervision during limited times for lunch.
3. All property, inside or outside of the building, needs to be respected:
   • No writing on walls, furnishings, etc. inside or outside of the building.
   • No eating food or drinking on carpeted areas of all buildings.
   • No gum chewing on campus.
   • Remove food and trash from the building or the grounds.
4. Academy Day students must be attended by an adult at all times.
   • Parents who are working during assembly time should place their students under the direct supervision of another parent.
   • After the opening assembly, students should not enter the top story of the main building (including the nursery) without adult supervision.
5. If a student does not have an Academy Day class, they must remain in the lounge or with a parent.
6. Teen helpers need to stay in the assigned class. When necessary, they should run errands promptly and return immediately.
7. Children in Nursery to 5-7’s should be picked up by a parent, another adult, or teen sibling.

Liability Release

In consideration for being accepted by South Bay Faith Academy, Inc. for participation in all school/family field trips and activities that take place off campus, we (I), being 21 years of age or older, do for ourselves (myself) (and for and on behalf of my child-participant if said child is not 21 years of age or older) do hereby release, forever discharge and agree to hold harmless South Bay Faith Academy, Inc. and the directors, employees, and agents thereof from any and all liability, claims or demands for personal injury, sickness, death, personal or property damage and attorney fees and/or expenses, for any nature whatsoever, no matter the cause, which may be incurred by the undersigned and the child-participant that occur while said child is participating in the above-described trip or activity. It is understood and agreed that South Bay Faith Academy, Inc. is not covered by insurance for all school/family field trips that are off campus. All responsibility lies with the undersigned and/or the child participant.

Furthermore, we (I) (and on behalf of our (my) child-participant if under the age of 21 years) hereby assume all risk of personal injury, sickness, death, personal or property damage, and attorneys’ fees and/or expenses, as a result of participation in recreation and work activities involved therein.

The undersigned further hereby agree to hold harmless and indemnify said academy, its directors, employees, and agents, for any liability sustained by said academy as the result of the negligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto.

Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, we (I) hereby assume all transportation costs.

(If under 21 years of age, both parents must sign unless parents are separated or divorced in which case the custodial parent must sign.)
