I acknowledge that the nature of a cooperative means "all hands on deck" and that I will be required to teach or help in order to make Del Rio Homeschool group to function. We do our best to provide each parent with at least one class period off but are limited by circumstances such as illness resulting in multiple family absenses. We appreciate a willing attitude and a servant's heart when it comes to this matter. Leadership spends a good deal of personal time coordinating the entire schedule of helpers/teachers for each co op day and we appreciate each participating parent! We reward willing teachers by giving them priority over non-teachers for class registration and special events.

I release, waive, discharge and covenant not to sue DEL RIO HOMESCHOOL GROUP, its affiliated clubs, their respective administrators, directors, agents, and other employees of the organization, other members/participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors or premises used to conduct the event, all of which are hereinafter referred to as “releases,” from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death and damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise. This is required for you to check this box to attend any co-op function and takes the place of the separate form from previous years.

ADULT AND CHILD WAIVER AND RELEASE OF LIABILITY In consideration of being allowed to participate in any way in DEL RIO HOMESCHOOL GROUP related events and activities, the undersigned: Agree that the member/participant should inspect the facilities and equipment to be used, and if the member/participant believes anything is unsafe, he or she should immediately advise supervisor (advisor, manager, etc.) of such conditions and refuse to participate. Acknowledge and fully understand that each member/participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence but the action, inaction, and negligence of others, or the condition of the premises or of any equipment used. Further, I understand that there may be other risks not known to us or not reasonably foreseeable at this time. I the parent, assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death to the child being signed up at this time and myself.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

A. Assumption of Risk/Waiver of Liability/Indemnification Agreement

In consideration of being allowed to participate on behalf of Del Rio Homeschool Group (DRHG) and related events and activities, the undersigned acknowledges, appreciates and agrees that:

1.) Participation includes possible exposure to and illness from infectious disease including but not limited to MRSA, influenza, COVID-19, and a myriad of other communicable diseases. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2.) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3.) I willingly agree to comply with the stated and customary terms and conditions for participation as outlined in the COVID 19 Policy regarding protection against infectious diseases. If, however, I observe or become aware of any unusual or significant hazards during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest board member immediately; and,

4.) I, for myself and on behalf of my child(ren), wards, heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, SAVE and HOLD HARMLESS DRHG, their officers, officials, agents, volunteers, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the events (“Releasees”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY WITHOUT ANY UNDUE INFLUENCE OR INDUCEMENT.
