This applies to all students ages 4 and up. Students have breaks between each class, therefore unless they have extenuating circumstances they should be able to remain in classes throughout the duration of the class period.

The other must be a NON-Relative that has worked with your children in an organized capacity (Sunday school teacher, cub scout leader, sports coach, piano teacher...) If your non-realtive reference has not worked with all your children, list additional non-relative references for each of your children.
