Handbook Policy Notice and Family Participation Agreement

Given my intent to participate with and enroll my child(ren). (enter names in the field below)_ in the Campbellsville Homeschool Co-op during the 2025-2026 academic year, I, _(enter in field below) , hereby acknowledge receipt of the Campbellsville Homeschool Co-op Handbook. Furthermore, I acknowledge that I have reviewed the enclosed Mission Statement and Statement of Faith, as well as the Discipline, Zero Tolerance, and Bullying policies. By my signature below, I agree to abide by the Handbook and to support and adhere to the aforementioned statments and policies. I further commit to cooperating in good faith with the Advisory Board in any disciplinary processes, to guiding my children in complying with the handbook, and to bringing issues or questions related to these policies to the Board for consideration and peaceful resolution. I acknowledge that all Board answers, actions, decisions, and resolutions are final and binding. I (on behalf of my entire family) hereby release the Co-op and its board members (past, present, or future) from any and all liability arising from and/or relating to my familiy's participation in the Campbellsville Homeschool Co-op.  In addition, I agree to release and hold harmless Green River Memorial Baptist Church from any and all claims, liabilities, losses, damages and actions directly or indirectly arising out of my use of their facilities.
