Guest Form

Reasons to Use this Form:

1) A non member bringing a student to co-op (rare occasion) with notification of Admin Board

2) A non member attending a day of co-op with a member with notification of Admin Board

3) A Guest Teacher approved by the Admin Board (not a Non-Member Teacher which is a separate form)

4) A Member who does not have a background check on file

Release for Liability for Guests

(1) In consideration for being invited to attend and participate in GRACE Homeschoolers activities as a Guest of a GRACE Homeschoolers member, we (I), being 18 years of age or older, do hereby release, forever discharge and agree to hold harmless GRACE Homeschoolers and its affiliates and members and those responsible for the facilities used by GRACE Homeschoolers and its affiliates, members, directors, teachers, employees, volunteers, and agents thereof from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses of any nature whatsoever which may be incurred by the undersigned and/or the child(ren) participating in the above described classes and activities.

Furthermore, I hereby assume all risk of personal injury, sickness, death, damage, and expense as a result of participation in recreation, volunteer and/or work activities involved therein.

The undersigned further hereby agrees to hold harmless and indemnify said classes and church, its members, directors, teachers, employees, and agents for any liability sustained by said classes and church as the result of negligent, willful, or intentional acts of said parent, guardian, and/or participant, including expenses incurred attendant thereto.

(2) I agree while I am on campus I will conduct myself in a manner that is appropriate to a Christian family environment and will be respectful of the views, core beliefs, and policies of GRACE Homeschoolers.
