I, the undersigned, am the parent or legal guardian of the minor(s) participating in Koinonia Homeschool Hybrid Academy ("Koinonia"). I acknowledge that Koinonia will be using indoor and outdoor facilities of Faith Bible Fellowship Church of York, PA (the "Church").

Church Acknowledgment:

I acknowledge the following:

A. The Church does not sponsor Koinonia.
B. The Church does not control the curriculum, program, or activities of Koinonia.
C. The Church provides no supervision of Koinonia's leaders, employees, agents, volunteers, or participants ("Koinonia Personnel").
D. The Church does not conduct or review background checks on any of Koinonia's personnel.
E. The Church does not charge a fee for the use of its facilities; any donation made by Koinonia is completely voluntary.
F. The Church does not actively monitor the conditions of its facilities to ensure they are free from hazards during Koinonia’s use.

Assumption of Risks:
I further acknowledge that the minor will be participating in Koinonia's programs and activities at the Church’s facility with the full understanding that such programs and activities may involve significant risk of bodily injury, illness, death, or property damage. These risks include, but are not limited to:

• Harm from defective premises or conditions

• Uneven terrain

• Weather conditions

• Contact with plants, insects, or contagious diseases

• Physical exertion from activities such as running and jumping

Assumption of All Risks:
Knowing and understanding these material risks, and reasonably anticipating that injuries, illness, and even death are possible, I hereby expressly assume all the risks of injury, illness, harm, death, or property damage that may occur to the minor in connection with Koinonia's use of the Church's facilities.

Indemnity, Waiver, and Release:
In consideration of the benefits to the minor from Koinonia’s use of the Church’s facilities, I agree, for myself, any co-parent or guardian, the minor, and the minor's representatives, executors, heirs, agents, and assigns, to waive, release, indemnify, and hold harmless Faith Bible Fellowship Church of York, PA, its elders, officers, agents, and employees, from any and all liability, actions, causes of action, damages, claims, or demands of any kind, including but not limited to claims for negligence or any other form of legal action, including attorneys' fees, arising from the Church’s facilities or participation in Koinonia's programs or activities.

Certification:
I hereby certify that I am voluntarily signing this minor’s Indemnity, Release, and Waiver and intend to be legally bound by the terms of this document. I have read all of its provisions and fully understand its significance.
