Liability Waiver and Consent Form

PLEASE READ CAREFULLY BEFORE SIGNING

Thank you for being a part of Nolichuckey Baptist Homeschool Academy (NBHA). We are pleased to offer your child a variety of experiential learning opportunities that may not be available in traditional educational settings. However, participation in these activities carries inherent risks, and NBHA, along with Nolichuckey Baptist Church, cannot assume liability for any accidents or injuries that may occur.

By signing below, I/We, the undersigned parent(s) or legal guardian(s):

Consent for Participation
I/We grant permission for my/our child to participate in all NBHA-related activities throughout the academic year. I/We understand that if I/We wish to exclude my/our child from any specific activity, I/We must notify NBHA staff in writing in advance.
Assumption of Risk and Waiver of Liability
I/We acknowledge and assume all risks and hazards associated with my/our child’s participation in NBHA activities, including but not limited to transportation to and from school, field trips, and other events. I/We hereby waive, release, indemnify, and hold harmless NBHA, Nolichuckey Baptist Church, its staff, volunteers, and any guest instructors or facilitators from any claims, liabilities, or damages arising from my/our child's participation.
Medical Responsibility
I/We understand that NBHA and Nolichuckey Baptist Church do not provide medical insurance for my/our child. Any medical expenses incurred while my/our child is in the care of NBHA remain my/our sole financial responsibility.
Emergency Medical Authorization
In the event of a medical emergency requiring immediate attention, I/We authorize NBHA staff to make decisions regarding necessary medical care for my/our child, including but not limited to CPR, wound care, emergency transportation, and the administration of medication for acute medical conditions such as severe allergic reactions or snake bites. Every reasonable effort will be made to contact me/us prior to any medical intervention whenever possible.

Academy Policies

Class policies establish objective standards that promote learning and foster respect for others. We strive to uphold Christian values by defining necessary boundaries (Psalm 119:44-45). Below is an introduction to some key policies.

If you have any questions regarding these policies, please do not hesitate to ask.

As a private academy, we reserve the right to accept, reject, or expel any student at our discretion.

All students must be enrolled under Homelife Academy.
We will use the Team Reach app as our primary means of communication with parents and for sharing important information.

Conduct and Manners

“I will behave myself wisely in a perfect way.” (Psalm 101:2)

All students are expected to:

Obey authority figures (parents, teachers, director, etc.), even when they are not immediately present. Such obedience should be willing, cheerful, and immediate.
Take responsibility for completing assigned tasks.
Show courtesy and respect toward students, teachers, and visitors.
Maintain personal and property cleanliness.
Respect the property of others and the school.
Uphold moral conduct in recreation and social interactions.

Discipline

The goal of our discipline policy is to maintain an atmosphere conducive to learning. Teachers will clearly communicate classroom rules on the first day and consistently enforce them throughout the year. Specific disciplinary procedures will be discussed with parents during the admissions interview to ensure a shared understanding of expectations.

While not an exhaustive list, the following offenses may result in suspension or expulsion:

Possession or use of tobacco products, vaping devices, alcohol, marijuana, illegal drugs, pharmaceuticals, or controlled substances
Possession of any type of weapon
Lying, stealing, or cheating
Disrespecting authority
Destruction of property (damaged academy/church property may incur charges)
Persistent classroom disturbances

Dress Code

“Whatsoever ye do, do it heartily, as to the Lord, and not unto men.” (Colossians 3:23)

Attire reflects attitude and values. We expect students to dress in a manner that aligns with the values we teach. Professionalism in dress fosters a focused learning environment, while modesty reflects a desire to honor the Lord rather than seek attention.

Dress Code Requirements:

Students must wear the designated Church School shirt paired with khaki, olive, navy blue, or black pants.
Girls may wear skirts in these colors, which must be loose-fitting and hemmed to the knee.
Boys may also wear shorts in these colors, loose-fitting, and to the knees

Arrival and Dismissal

Arrival: Students should arrive by 8:30 A.M. and enter through the front door into the sanctuary. They must be seated and ready for morning devotions at that time.
Dismissal: Classes dismiss at 1:55 P.M. All students will return to the sanctuary to wait for pickup. Parents should collect their children at the front door.

Attendance

We understand that unexpected circumstances, such as illness or a family emergency, may cause absences. Up to three unplanned absences per semester are allowed. Beyond this, the student will be placed on a probationary attendance plan. Excessive absences may result in non-readmittance. All missed homework must be completed and submitted in a timely manner.

Pre-Arranged Absences

Pre-arranged absences may be approved at the director’s discretion.
Students may be excused for medical or dental appointments or personal reasons with a signed note from a parent, which must be given to the homeroom teacher or director.
No student may leave the building during school hours without permission from the homeroom teacher or director.

School Closings

Notifications for inclement weather closings will be sent through the Team Reach app as early as possible.
If an early dismissal is necessary, parents will receive a text message alert in addition to the Team Reach announcement.

Recess During Harsh Weather

Recess is held outdoors unless weather conditions pose a health or safety risk. Students with specific needs may stay indoors.

Devotion and Assemblies

Morning devotions take place from 8:40 A.M. to 8:55 A.M. in the sanctuary.
All students must attend and sit with their class in designated pews.
Upon entering the sanctuary, students must remain silent, reverent, and attentive throughout the devotion service.

Tuition

Tuition or the first three monthly payments must be paid up front before the first day of classes.
Tuition payments are non-refundable.

Grading System

All teachers will maintain records of each student’s progress. Reports will be sent home at the end of each semester.

Grades Pre-K – 2nd:

E – Excellent
S – Satisfactory
N – Needs Improvement
U – Unsatisfactory

Grades 3rd – 12th:

A – Superior (94% – 100%)
B – Above Average (86% – 93%)
C – Average (78% – 85%)
D – Below Average (70% – 77%)
F – Failing (Below 70%)

Report Cards

Students will receive report cards at the end of each semester (approximately 30 school days). Additional reports may be issued if a student is struggling in a subject.

Medications

All medication must be stored and administered in the school office.
Medication must be in its original container with a current date and accompanied by a written order from a physician or a note from a parent.
Medication should only be administered at school when absolutely necessary (e.g., medication prescribed three times a day should be taken before school, after school, and at bedtime at home).

Lunch

Lunch and snacks will be provided by the school each day.
Students may bring their own lunch if they prefer.

Use of Telephones and Electronics

Students may not use personal phones or electronic devices during class times.
Students may use the school phone only with permission.
Students will not be called from class for phone messages unless it is an emergency.

Nolichuckey Baptist Homeschool Academy
Parent and Student Statement of Faith

A. The Bible

We believe the Bible, consisting of the Old and New Testaments, is the inerrant and infallible Word of God. It was given by divine inspiration and serves as the only certain and authoritative rule for every aspect of the Christian life.

B. The Trinity

We believe in one true and living God, who is triune in nature, manifested as God the Father, God the Son, and God the Holy Spirit. In all things, we owe Him our highest love, reverence, and obedience. The members of the Trinity have distinct individual attributes but are united in essence, character, nature, and being.

C. God the Father

We believe there is only one God, who created, sustains, and rules over the universe. The historical account of creation in Genesis affirms that God is the personal and direct Creator of all that exists, including the first humans, Adam and Eve, from whom all humanity descends.

D. God the Son: Jesus Christ

We believe Jesus Christ is the second person of the Trinity and the eternal Son of God. Conceived by the Holy Spirit and born of the Virgin Mary, Jesus lived a sinless life, perfectly revealing and fulfilling the will of God. His sacrificial atonement on the cross provided redemption for sinful humanity. He was crucified, rose on the third day, and ascended to the Father. He alone is sufficient as Savior and reigns as the Sovereign Lord of the universe. Seated at the right hand of the Father, He intercedes for believers as the only mediator between God and humanity. As Scripture declares, in Him “dwelleth all the fullness of the Godhead bodily” (Colossians 2:9 KJV), and He is “over all, God blessed forever” (Romans 9:5 KJV).

E. The Holy Spirit

We believe the Holy Spirit is the Spirit of God and fully divine. He convicts individuals of sin, righteousness, and judgment, and enables believers to understand God's truth as revealed in Scripture. The Holy Spirit indwells and empowers believers for godly service, worship, and witness.

F. Humanity

We believe God created humanity in His own image, and Adam and Eve were originally created in perfection. Through Satan’s temptation, they fell from their sinless state into disobedience, bringing sin upon all humanity. As a result, all people are sinners, fall short of God’s standard of perfection, and are under divine condemnation. The only remedy for sin is personal salvation through faith alone in Jesus Christ, wholly apart from human merit and works.

We also believe that marriage, according to Scripture, is the union of one man and one woman. Furthermore, we affirm that God forbids any form of homosexuality, lesbianism, bisexuality, incest, fornication, adultery, and pornography.

G. Salvation

We believe salvation involves the redemption of the whole person and is freely offered to all who accept Jesus Christ as their personal Lord and Savior through repentance and faith. Salvation is entirely by God’s grace and cannot be earned through human effort. However, it must be personally received through an individual's free choice, apart from any human merit. Salvation is not possible apart from faith in Jesus Christ, and those who die without accepting Him as Savior will face eternal separation from God in everlasting torment.

H. The Church

We believe Jesus Christ is the Head of the Church, which is composed of all true believers in Him. Christians are called to associate themselves with local churches, faithfully serve Christ, and fulfill the Great Commission. Each local church has the authority, granted by Jesus Christ, to govern itself, maintain order, worship, and carry out its ministries.

I. Life of the Believer

We believe Christians should live in accordance with Scripture, demonstrating integrity in character and conduct. The Bible is our supreme authority and provides the moral and ethical foundation for personal behavior both within and outside the academic community.
