Please complete the application below. Add all children who wish to participate in co-op this year. There will be forms to complete on the second page of this membership request. All forms must be completed before membership will be considered. The Participant Handbook is on the main home page. Membership Info> Participant Handbook. Please be sure to read that before submitting your application.

**On the second page, please complete the list of forms first. Once all forms are complete, there will be a message letting you know you have completed all the forms. Close out of that window, then you can follow the link to pay. ** If this gets done out of order, we will have to send those forms back to you to sign/complete again so they will link with your account.

Once the application and all attached forms are complete, your application will be reviewed and a determination email will be sent.

Payment Instructions

This $10.00/per family is for access to our website/app for class registration, communication, sharing photos, & calendars of events this year. This is not an additional fee, and will be deducted from total registration invoices. This $10.00 communication fee must be paid within 5 days, or your family account will be parked automatically. If this happens please contact Sarah or Lisa to resolve that and have your account active.
