2026 Prom Waiver

Parents, please read our Frequently Asked Questions page before signing the waiver below permitting your teen(s) to attend the SEICHE PROM 2026 at The Clayton Batchelor Pavilion in Aurora IN, 7:00-11:00 P.M.

SEICHE Prom Transportation and Exit

It is the responsibility of the attendee’s guardian to arrange transportation to and from the prom-event. It is also the responsibility for an attendee’s guardian to arrange transportation for any after-prom activities. Guardian needs to notify chaperones if attendees are leaving prior to 11:00pm.

I, the undersigned parent or legal guardian of above named student, hereby grant permission for my
student to participate in the Seiche Prom Event 2026, which is scheduled for May 1, 2026 from 7:00-11:00 pm at the Clayton Batchelor Pavilion in Aurora IN.

I have reviewed the food menu and informed the event organizer of any severe food allergies or limitations. I am also aware that all food items are made in a facility that is not gluten-free, nut-free, or dairy-free. Desserts are packaged with other desserts containing nuts. SEICHE and the catering provider cannot comfortably guarantee an absolutely GF, nut-free, or dairy-free meal for severe medical allergies. SEICHE and the catering company cannot provide individual prepared meals.

I understand that it is SEICHE’s desire to provide an enjoyable and safe environment for the students attending the Prom.

I understand that photographs and/or videos may be taken at the event and may be used in promoting future events. This includes on social media platforms.
