All YHSA teens ages 14 (as of 4/25/2026) through 2026 Senior are invited to attend the 2026 Spring Formal! Each teen may bring up to 1 guest. The guest MUST be ages 14 (as of 4/25/2026) through 2026 Senior.

To register: FIRST, read the Dress Code, Code of Conduct, and Parent Medical Release at the bottom of this info page. Teens and parents will have to agree to abide by both codes and parents will have to agree to the parent medical release. You will be asked to indicate your agreement by typing your name in the electronic signature boxes on the sign-up form. Please take these consent forms very seriously. Doing so acts as your signature and will be used as such if an issue should arise. For a printable form of each of the documents, simply click on the hyperlink for that document.
Once you have read both of the codes and the parent medical release, click on the "sign-up" link below to fill out the online form.

Payment:
YHSA Members: payment can be made at the time of registration. If you do not pay when you register, the amount due will be added to your YHSA account (you MUST be signed into your account in order for this to happen!). Balances are due by Saturday, 3/21/2026.
Guests: you will make your own payment and when the form is submitted, you will automatically be directed to the payment page.
If a teen member is going to pay for their guest, the guest will register themselves and choose the "I am a guest of another teen that is paying for me" price option.

Music Requests:
Each attendee will have the opportunity to request 1 song during their registration. This song should not be a "slow" song and cannot contain inappropriate content or language (no profanity, no alcohol, drug or sexual references, etc). All song requests will be reviewed and if found appropriate, added to our Spring Formal playlist.

"Slow" songs will be voted by you! We have compiled a list of what was played at last year's event and what requests came in at the event. You can choose 3 songs but be aware that you can only vote for a song once; if you vote for the same song all 3 times, it will only count as 1 vote.

Volunteers:
In order to host this event, we need parents (or grandparents) to volunteer! All volunteers must be YHSA members and have their clearances on file with YHSA. The clearances needed are: Pennsylvania Child Abuse History Clearance, PA State Police Criminal Background Check, and the FBI Clearance. If you don't have these, you can get them by visiting our Pennsylvania Clearances for Volunteers page (click HERE) and choose the option that fits (Resident less than 10 years or Resident 10 years or more). Clearances should be sent to our Clearance Organizer (yhsaclearances@gmail.com) before April 15th.
Chaperones: chaperones will need to be there from 5:00pm until 10:00pm. They are responsible for assisting with check-in/registration at the beginning of the event and for helping keep an eye on all areas of the venue throughout the event. Chaperones must be parents or grandparents. We need a total of 15 chaperones.
Photographers: Photographers will need to be there from 5:00pm until 10:00pm. They will work together to ensure we get pictures of all the different areas prior to the doors opening for the attendees. As attendees enter, they will take pictures of them coming in. They will move around the venue throughout the event to ensure we get as many photos of the event as possible. Photographers will need to have their own equipment. We will have one "album" for all attendees to view; login information will be sent to the photographers so they can upload the pictures to the album. Photographers must be 21 or older. We need a total of 3 photographers.
Event Clean Up: We need volunteers to come early to help us get everything cleaned up. These volunteers will arrive at 9:30pm to help get the downstairs areas cleaned up. The event concludes at 10:00pm. At 10:00, these volunteers will assist with cleaning the upstairs areas and loading out everything. We must have everything cleaned up and be out of the venue by 11:00pm (no exceptions).

ALL ATTENDEES:
- Dress to high standards of modesty and respectability in apparel ranging from semi-formal to formal attire.
- Jeans are NOT permitted
- Shoes must be worn at all times.
- By participating in YHSA’s dances, attendees agree to abide by the Christian standards set forth by the group. Attire should match God-given gender.
GENTLEMEN:
- Semi-formal attire such as dress pants and button-down shirts with tie/bowtie may be worn.
- Button-down shirts must remain buttoned and ties must remain tied around the neck of the attendee
- As an alternative, formal attire such as tuxedos or dress blues may be worn.
LADIES:
- All modest dress styles are permitted.
- Exclusions: bare midriff (abdomen and back), cut-out sides, exposed cleavage, strapless dresses, slits in skirt
- Sheer material (lace, chiffon, etc) is NOT acceptable coverage for exclusions listed above.
- All gowns must be BELOW the bottom of the knee or longer.
- Sheer material alone (lace, chiffon, etc) is NOT acceptable coverage above the knee.
- As an alternative, a woman's pant suit or woman's dress pants with a fancy top (that is of code) are permitted to be worn.
If you are uncertain about attire, you can reach out to the YHSA Events Chair or any YHSA Board Member.
We want to represent YHSA and what we stand for in our bylaws. Please respect them. If you don’t adhere to the afore mentioned dress code, you will not be admitted and parents will be contacted. We love our families and kids and want to guide them the best we can.

All teens attending this event, including guests, will conduct themselves in a Christ-like manner. This means obeying all adults and chaperones, respecting the property, and treating other attendees with respect and dignity. The use of tobacco, alcohol, illegal drugs, and profanity are expressly forbidden.
No outside food or drinks will be allowed into the venue. No bags or backpacks larger than small purses will be allowed past the registration area. Any larger bags will be secured until the end of the dance. All pockets and smaller bags and purses will be checked through respectfully at check-in.
**Violating the Code of Conduct will be grounds of immediate expulsion from the event. **
PLEASE NOTE: NO refunds will be given! If you are denied admission to or are expelled from the event due to a code violation, then you will not receive a refund.
***No Minor will be permitted to leave the event before 9:30pm unless signed out by a parent or guardian. ***

I hereby give YHSA and its representatives permission to seek emergency medical treatment for my child (as named on the registration form) should it become necessary while he/she is attending the 2026 Spring Formal. I have included a phone # where I can be reached in the event of an emergency. I release YHSA and its representatives of any liability while my child is at the 2025 Spring Formal.

First Name/Last Name - you are signing up on the public form and will need to enter the first and last name of the teen that is attending.
Individual Price Option - This automatically populates as 2026 Spring Formal Attendee - $65.00. If you click this box, it gives you the option to change it to 2026 Spring Formal Attendee + Guest - $130.00 or I am a guest of another teen that is paying for me - $0.00. Please choose the correct option.
Are you a YHSA member or guest - Click the box and choose the correct option.
Attendee's first and last name - This is the name that will be used for registration. Please type the name of the teen that you are registering in this box.
Attendee's birth date - This is the birth date of the teen that you are registering. Please type it as a 2 digit month/2 digit day/4 digit year (ie 01/01/2026).
Attendee's age as of 4/25/2026 - This is the age of the teen that you are registering as of the day of the dance.
YHSA Members are you paying for a guest? - Click the box and choose the correct option.
If you answered yes, what is the name of your guest? - Please type the first and last name of your guest. If you are not inviting a guest, please type NA. This field is informational only. It does not register your guest for the dance; it just lets us know that you are inviting someone and to be on the lookout for their registration.
My guest must register themselves in order to be added? - This is an another informational field, reminding you that if you are inviting a guest, they must use the link provided in the first drop down option and register themselves. Click the box and choose the correct option.

Song Request - if you would like to request a song to be played at the event, please type the song title and artist here. All song requests will be reviewed for content and time and those that are approved will be added. Please note, while we try to add as many as possible, just because you request a song does not mean that it will be added.
Vote for a slow song (3 questions) - choose the options from the list that you like. If you choose the same song more than once, it will only count as 1.
What is your favorite book/book series? - please type the title and author of your favorite book or book series.
