Before joining HIM please note that HIM membership is for families who meet the following:

At least one parent/guardian from each family must affirm (state as fact; assert strongly and publicly) the HIM Statement of Faith and any parent/guardian from the same family serving in a leadership position
Be a home educating family in which a parent/guardian is the primary educator of at least one child in kindergarten (age 5+ on or before Aug 31) through grade 12 (this excludes virtual public schools such as OHVA)

When completeing the form you may leave the child email/username field(s) blank, but you must list each child (including birthdate) who will be participating in events, activities, and co-op as this information will be used for sign-ups/registrations.

By joining HIM I hereby release and forever discharge Homeschooling In Marysville (HIM), its administrators, agents, assigns, and all other persons, firms, corporations, and educational institutions, who it might be claimed to be liable, none of whom admit any liability from any and all claims, demands, actions, causes of action or suits of any kind or nature whatsoever and particularly on account of all injuries known and unknown, both to person and property, which have resulted or may in the future develop from any accident which might occur as a result of any social, educational, athletic or any other event or activity sponsored by HIM. I hereby declare that the terms of this settlement have been completely read and are fully understood and voluntarily accepted for the purpose of making a full and final compromise adjustment and settlement of any and all claims, stipulated or otherwise, on account of the injuries and damages above mentioned, and for the express purposes of precluding forever any further or additional claims arising out of any possible accident..

It is further agreed that the release expresses a full and complete settlement of liability, regardless of the adequacy of the aforesaid and that the acceptance of this release shall not operate as an admission of the liability on the part of anyone, nor as estoppel, waiver, or bar with respect to any claim the part or parties release may have against me.

I agree that it is the parent’s responsibility to provide for their child(ren)’s own accident and health coverage while participating in any functions with HIM and I acknowledge that HIM does not provide this coverage.

Furthermore, I fully understand and acknowledge that HIM is not responsible for any personal property lost, damaged, or stolen while participating in any functions with HIM. This release is binding on my heirs, executors, assigns, and administrators.

This is a voluntary release for any and all future injuries or accidents. I am fully aware of the risks of attending, traveling to, and participating in social, athletic, and all other events and hereby assume all risks. The risks include those foreseen and unforeseen, known and unknown.

Any claim or dispute arising from or related to this agreement shall be settled by mediation and, if necessary, legally binding arbitration in accordance with the Rules of Procedure for Christian Conciliation of the Institute for Christian Conciliation, a division of Peacemaker® Ministries. Judgment upon an arbitration decision may be entered in any court otherwise having jurisdiction. The parties understand that these methods shall be the sole remedy for any controversy or claim arising out of this agreement and expressly waive their right to file a lawsuit in any civil court against one another for such disputes, except to enforce an arbitration decision.
