Athletic Forms

Please print these forms, fill them out, and bring them with you to Registration for your sport

1. A Copy of the student's birth certificate
2. A copy of educational transcript for new Varsity Players
3. Athletics Liability Form - must be filled out once per year
4. ACEL Homeschool Rule - must be filled out once per year
5. Parents and Players Code of Conduct - must be filled out once per year
6. Physical Form - must be signed by a physician and given to EACH coach 
7. CHA Membership Form and fee - if not already registered or renewed  (Online preferred -- www.nelcha.org)
8. Academic Eligibility - to be filled out as a report card throughout the season when requested by the coach
9. 12 and Under Registration Sheet - for Basketball 12 and Under Players Only
10. Bring at least 2 checks.  One for registration fees and one for uniform deposit (postdated to the end of the season)
