  1. Athletics Liability Form (fill this out online by clicking here) 
  2. CHA Registration Form and fee (Registration for the 26-27 school year will open 7/1/26 - if new to NELCHA please fill out the registration form and put in the notes that it is for the 2026-27 school year)
  3. Physical Form - must be signed by a physician or nurse practitioner - must be dated after 04/01/2026
  4. Inital $75 is due with registration & remaining $75 is due May 1st (can pay by Paypal friends and family -- paypal.me/chaathletics -- or Venmo @chaathletics please comment your player's name)
