Ordering the PASS test through HSA
PASS TEST ORDERING INFORMATION
Order Deadline: April 13
Cost: $28 per test (includes shipping, non-refundable after April 13)
The PASS Test is a non-timed, standardized test developed for homeschool students in grades 3-8, and
is designed for parents to administer at home. The test covers Reading, Mathematics, and Language.

Please read the following information through carefully (2 pgs) for important details regarding test
materials, pick up/return dates, and procedures.
I will be coordinating the orders for both PCC LEAH and HSA LEAH. Please read the information carefully to know which will apply to one of the groups and what will apply to everyone.
How to Order:
Please mail the following 2 items to Stephanie Walton at address listed below no later than April 13:
1. Check made payable to Stephanie Walton (both groups)
2. Business size SASE (Self-Addressed Stamped Envelope) to receive your test results
3. Fill out the Google form using the link below (you will probably need to right click on link and click ‘open link’)

Test Pick Up:
Expect the PASS Test to arrive in late April, at which time you will receive an email from me letting
you know that your test materials are ready for pick up. Tests will be available to pick up during PCC LEAH Friday co-op or HSA on Tuesday. Please watch for the arrival email. If you are unable to pick up your test(s) at that time, you will need to contact me ASAP to arrange a time to pick up your materials from my home.
Administration:
You will have approximately 1 month to administer the test before all materials must be returned. I will
send a reminder email as the return deadline approaches.
Returning Test Materials:
Please plan on returning your test materials to PCC LEAH Tuesday May 22 - Thursday May 24 between 9 am - 12 noon (this is during end of year testing) or at HSA on Tuesdays unless clases have finished then you will have to contact me or drop it off at my house, you can also bring them to the HSA Science fair/History Jamboree on May 15. There will be a marked collection crate in the foyer.
Items received in your package:
You will receive a test scan sheet for each student, instructions, a disposal verification form, and ONE
SET of books: Reading, Language, and Math. If your family has students in both lower math (grades 3-
5) and upper math (grades 6-8), you will receive both levels of books.
Items to return no later than Thursday, May 24:
- Completed Test Scan Sheets
- Signed Disposal Form
- Any unused Test Scan Sheets
Books do not have to be returned.
However, test scan sheets cannot be processed without a signed disposal form verifying that your family has disposed of the books. Please be sure to include the signed disposal verification form
when returning your materials.
Please note that in order to qualify for group pricing, we must return all tests as a group. Therefore, after Thursday May 24th you will need to mail your own forms to Hewitt for individual processing and will be assessed an additional $10 per test fee by the company. If you cannot make the deadline for any reason, please contact me to let me know so that the group order is not delayed.
Test Results:
Results should arrive within 4 weeks (typically late June) and will be mailed to you as soon as possible in the SASE you provided with registration.

What is the pricing of a group order?
- with most group orders this one will offer a discounted price if we order a minimum number of tests. Since we don’t know when starting out if we will meet that minimum order we charge the full price. If we meet that group price we will pay less per test, this will create a refund for each test ordered. We have come up with 2 options for our refunds. Option 1: if the refund is less than 4.99 per family we will put that money into the scholarship fund for your respective group. Option 2: if the refund is 5.00 or more per family you may choose to put it toward your groups scholarship fund or receive a check for that refund when I send your results. I know this is a little confusing if you need more clarification please give me a call.
