I understand by submitting this First Class Kitsap County Homeschool Co-op membership form that our family is participating in co-op activities at our own risk, and therefore we release the Leadership team, First Class Kitsap Co-op and their host church from all liability. 

I have submitted our About Our Family information at registration.

I have read FCKC Attendance Policy.

I also understand that being a member of FCKC Co-op involves responsibility, and I acknowledge I have read the Parent Handbook (under the Membership tab on the horizontal navigation menu) and will adhere to the guidelines set forth within parent/child responsibilities section of the catalog to the best of my ability.

I have read FCKC Faith Statement found in our handbook. To attend our co-op, you have to agree to not undermine our Faith Statement in any way. If you choose to teach, you must be a Believer and must agree with our Faith Statement and sign it.  (RECAP: When offering to teach, you will sign our Faith Statement. To attend our co-op you do not need to sign our Faith Statement, but agree to not undermine it in any way. Doing so will result with immediate removal of co-op and all fees paid will be forfeited.)

I also understand and give permission for FCKC to use pictures/images of my family members or myself in our newsletter, website, Facebook and for all promotional materials. I understand that these pictures/images help to tell the story of Homeschooling and our co-op and I release and hold harmless FCKC, its photographers or agents, from any and all claims and demands associated with this release, including all claims for libel. I also waive any claim for damages and any right to compensation that I may have for the foregoing.
