This is the form you will fill out to purchase a Student Ad in the CHOICE Yearbook. The cost for an ad is $10.00. We set aside 1-2 pages for student ads so your ad will not be a certain size. It will just fit on the pages. Please fill in the name of the student and what you would like for your ad to say. You may upload up to three photos to be used in your ad, but you are not required to upload any photos. After you click Submit, you will be taken to a page to pay for the ad using PayPal.
